Job SummaryDevelops and implements the bereavement program. Oversees bereavement plans for each patient and family. Provides group support and individual counseling to bereaved clients per bereavement plan of care.
Essential DutiesAssesses bereavement needs and provides bereavement follow-up to each bereaved referred to the Bereavement Program.Plans, develops, and implements Bereavement Support Groups and educational workshops on specific grief issues. Plans Memorial Service(s). Participates in Team meetings.Participates in in-service, staff and other agency meetings for the purpose of planning, coordinating and generally improving the quality of service provided by the agency.Participates in the orientation and training of new employees to the Bereavement Program. Develops educational materials and resources for distribution to families and staff.Ensures that required documentation for activities within areas of responsibility is completed in an accurate and timelyActively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.Performs other duties as assigned.QualificationsRequired
