**Job Description** To perform the following tasks and clinical functions in an efficient and cost-effective manner as assigned in order to help patients/families/significant others respond to their treatment needs; psychosocial screening and assessment, discharge planning, linkage to community resources, crisis intervention, patient/family counseling and psycho-educational philosophy, standards, objectives and policies of the Department and the Hospital Center. **Qualifications** + Bachelor's Degree from an accredited college or university' + Related experience in the social service and/or health care field, preferably one year in a medical setting + Bilingual - Spanish required SEIU 1199 at Mount Sinai Hospital, 955 - Broadway - Jackson Heights - OFFS, Mount Sinai Hospital **Responsibilities** 1. Gathers information on social problems through interviews with patients, families, community representatives and hospital personnel. 2. Helps patient in solving problem when problem and solution are clear and uncomplicated by psychological difficulties. 3. May assist patient/families on complex problems under close supervision 4. Helps patients/families to meet concrete needs such as obtaining services, housing, or employment. 5. May assist in obtaining medical equipment and support services. 6. Gathers information about community resources and services and makes appropriate referrals with liaison and follow-up 7. Assists hospital staff, patients, and families in discharge planning. 8. Perform specific assignments relevant to particular program. 9. The salary range for the role is $32.1155 - $36.7826 Hourly. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
