**Shift:** Full Time, Monday through Friday, day shift
**Job Duties:**
+ Evaluate patients' psychosocial status and readiness for transplantation
+ Assist patients in understanding and navigating the transplant process
+ Facilitate access to community resources and support services
+ Advocate for patients' needs within the healthcare system
+ Develop and implement psychosocial interventions to enhance patients' quality of life and success of transplant
+ Participate in multidisciplinary transplant team meetings
+ Document patient interactions and maintain accurate records
+ Provide crisis intervention as needed
+ Assist with end-of-life planning and decision-making processes
+ Communicates and collaborates regularly with multidisciplinary team
+ Assists in development of the discharge plan
+ Makes post-acute referrals and follow through to completion, including placements and transportation
+ Mitigates discharge delays through timely interventions.
**Transplant Social Worker Qualifications & Skills**
+ Experience working in a healthcare or medical setting
+ Strong communication and interpersonal skills
+ Ability to work effectively in a multidisciplinary team
+ Proficiency in crisis intervention and conflict resolution
+ Understanding of financial assistance programs available to patients
+ Competence in cultural sensitivity and inclusivity
+ Experience in patient education and advocacy
+ LMSW or equivalent clinical licensure
+ Master's degree in social work (MSW)
+ Minimum of 2-3 years of experience in a healthcare setting
+ Strong knowledge of psychosocial assessment and interventions
+ Excellent organizational and time-management skills
+ Ability to handle sensitive and confidential information
+ Empathy and compassion for patients and families
+ Proficiency in medical terminology and healthcare systems
+ Strong problem-solving and decision-making abilities
+ Commitment to ongoing professional development and education
