Job Title: RN Case Manager
Job Description:
The RN Case Manager is tasked with overseeing case management activities in alignment with company policies and ensuring proper documentation. This role involves managing complex and acute cases to guarantee that members receive high-quality, cost-effective healthcare services aimed at achieving optimal health outcomes.
Key Responsibilities:
Conducting initial assessments with patients and their families through telephonic case management.Performing intake evaluations with patients and their families.Ensuring families are well-equipped and supported.Creating personalized treatment plans that consider physical, financial, and other limitations.Modifying treatment protocols as necessary.Recommending and coordinating appropriate follow-up services to promote wellness.Arranging transportation for patients to and from their homes and relevant healthcare facilities.Collaborating with healthcare team members to discuss patient progress and determine suitable care plans.Providing cost-saving resources such as alternative funding programs, manufacturer assistance, and grants.Verifying that patients have adequate insurance coverage or financial means to afford their treatment.Developing innovative solutions or making appropriate referrals for patients unable to afford treatment.Maintaining confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPAA).Required Skills
- Active multi-state compact RN license including GA (compact license)
- Bachelors of Nursing (or higher)
- 3+ years of Case Management experience or clinical coordination of care
- 5+ years of direct patient care or bedside nursing or acute care
- Proficient with EMR and case management programs
Preferred Qualifications
CCM certification in preferred
Experience working in a managed care setting
Pay and Benefits
The pay range for this position is $36.06 - $38.46/hr.
