Job Description:

Full Time 40 weekly hours, day shift

Job Summary:

The Case Management Manager helps facilitate the management of the case management department in collaboration with Department Director. The Manager oversees a staff of case managers and social workers responsible for patient care coordination. Implements the developed case management programs including utilization review and intake or discharge planning. The Manager helps facilitate the management of the area through the direction of human and physical resources adjusting the daily operations to meet the changing needs/environment of the department. The Manager strives for excellence in patient care and patient care delivery by building a collaborative work environment among all staff and departments. The Manager supports an environment of healing and wellness and is an exemplar for compassion, acceptance, respect, and empathy. May cover cases as needed.

Principal Duties and Responsibilities:

The following is a summary of the major functions of this individual's job.  He/she may perform other duties, both major and minor, which are not mentioned below, and specific functions may change from time to time.

1. Leads the Cooperative Care Program

2. Manages the operational activities of the department

3. Responsible for Utilization Management activities

4. Supports and utilizes the concepts of performance improvement

Minimum Education, Training and Experience:

Required:

Graduate of accredited RN program.

Active Kentucky RN license required.

Minimum of five years clinical experience

Two years Case Management experience preferred.

Case Management Certification (ANCC, CCM or ACM-RN) required within 2 years of hire date.

Previous management experience required.
