The General Pediatrics Clinical Pharmacist will provide pharmacotherapeutic support to the pediatric hospitalist team, along with inpatient pediatric oncology patients. The General Pediatrics Clinical Pharmacist will also provide as needed coverage for tasks within the Valerie Fund Children’s Center. As such the role is expected to provide clinical care and support within both the inpatient and ambulatory settings. This pharmacist will participate in multidisciplinary committees and will also serve as a preceptor in the Atlantic Health System PGY1 Pharmacy Residency Program.
Additional responsibilities include:
Provide comprehensive pharmaceutical care to general pediatric and pediatric oncology patients, including: review and recommend optimal, evidence-based medication therapy; provide pharmacokinetic consultations as required; document any changes in patient and pharmacy records and inform others of medication changes; prevent, detect, monitor, document, and report adverse drug reactions and medications errors; participate in the drug therapy management of medical emergencies; promote the use of the formulary by converting non-formulary orders to formulary when possible. Participate in direct patient care rounding and activities in the inpatient setting. Support Pediatric Hematology/Oncology services in the ambulatory setting as needed. Facilitate transitions of care by obtaining patient medication histories as well as conducting patient education and counseling. Augment the education of healthcare and non-healthcare team members on treatments related to pediatric patients; serve as a preceptor and mentor for pharmacists, pharmacy residents and students. Participate in site-based and system-wide multi-disciplinary committees. Conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization with the goal of publication and/or presentation at local and/or national meetings. Function alongside PICU and NICU clinical pharmacy colleagues to identify and execute necessary updates for medication and medication-related orders within the electronic medical record.
