Job Title: Clinical Pharmacist Job Description The clinical pharmacist specialist is responsible for the provision of safe, effective, and prompt medication therapy. This position involves providing patient-centered care and integrated pharmacy services in a decentralized practice setting with physicians, nurses, and other hospital personnel. The specialist participates in all necessary aspects of the medication-use system while providing comprehensive pharmaceutical care to the patients in the assigned areas, supporting both centralized and decentralized medication-use systems. Responsibilities + Provide safe, effective, and prompt medication therapy + Deliver patient-centered care and integrated pharmacy services + Participate in all necessary aspects of the medication-use system + Provide comprehensive pharmaceutical care to patients in assigned areas + Support both centralized and decentralized medication-use systems Essential Skills + Pharmacy or Pharm D. from an ACPE-accredited School of Pharmacy + Clinical pharmacy experience in a hospital setting + Completion of an accredited PGY-1 Pharmacy Residency or fellowship AND PGY-2 Specialty Pharmacy Residency or fellowship (in an appropriate specialty field) + OR Completion of an accredited PGY-1 Pharmacy Residency or fellowship AND 3+ years of clinical pharmacy experience (if no PGY-2 is completed) + OR 5+ years of clinical pharmacy experience with no history of residency or fellowship + Board of Pharmacy Specialties (BPS) certification (or attained within 1 year) + Active Pharmacist license in good standing with the appropriate State Board of Pharmacy + BLS certification + ALS/ACLS certification Additional Skills & Qualifications + Doctor of Pharmacy from an ACPE-accredited college of pharmacy + Experience working with Microsoft Office applications + Experience with Electronic Medical Record Systems Pay and Benefits The pay range for this position is $130000.00 - $150000.00/yr. benefits start 1st day of the month after 30 days of employment Workplace Type This is a fully onsite position in Tulsa,OK.
