Clinical Risk Manager

JOB SUMMARY

\n

The Clinical Risk Manager (CRM) directs, develops and coordinates the clinical risk management activities for the hospitals. In doing so, the CRM will evaluate information obtained from the reporting of adverse events and near misses, Root Cause Analysis work, and communication relayed Risk Management. With this information, the CRM will review findings and will work collaboratively with Patient Safety and hospital leaders to develop responsive programs that will enhance an organizational culture that supports patient safety and reduces risk exposure.

LOCATION

This position will support two hospitals; Baylor Scott & White Medical Center - Lake Pointe and Baylor Scott & White Medical Center - Plano. (Travel to both locations is required)

\n

ESSENTIAL FUNCTIONS OF THE ROLE

\n

Patient Safety Information Management

\n\nReviews adverse event reports and obtains more detailed information by chart review and interviews as indicated.\nWhen adverse events cause harm to patients, the CRM under the direction of the Corporate Director of Risk Management, investigates in detail to evaluate degree of patient harm. When organizational risk is indicated, investigation of the event will be delegated to Corporate claims manager for further review. The CRM will participate, as appropriate, in the implementation of an early intervention.\nAddresses day to day questions and problems involving patient care or concerns. Addresses patient complaints or incidents and works to resolve issues before they develop into liability claims.\nReceives information from other team members who are accountable for complaint and Ethics Hotline management about specific patient care processes that may create risk of patient harm. The CRM will assist designated team members to address or resolve complaints and Ethics Hotline reports.\nAssists in addressing grievances that involve patient care and responds to the patient in the time frame designated by CMS.  Provides follow-up communication with the patient when the complaint is resolved.\nIs a trusted agent for clinical staff to report concerns about aspects of care that may generate risk to patients.\nParticipates in the effective disclosure of adverse events to patients and families by serving as a consultant and educator to those accountable to disclose this information.\nReports to BSWH department of Risk Management and facility senior clinical and executive managers a) the initial existence of the serious adverse events, concerns or risks and the plan for investigation; b) the results of the initial investigation; c) the plan for more detailed investigation (RCA or other) and d) the ?lessons learned? from study of the event.\nAssists the Manager of Insurance in managing claims related to property loss and visitor injury.\nClinical Risk Study and Priority setting that is based upon priorities and specific hospital performance.\nAssists in studying trends of adverse events and presents results to Hospital PS Committees and senior clinical and executive managers and, when requested, to governance groups.\nInvestigates and prepares for the root cause study of sentinel events to include the development of a timeline, contacting the required participants to establish their attendance, scheduling and facilitating the meeting, writes the report of the investigation, schedules a follow-up corrective action plan and monitors the plan. Sentinel events are reported to corporate risk management on a monthly basis.\nParticipates in the multidisciplinary adverse event review team that meets regularly to discuss adverse event reports.\nRegularly reports to executive and medical staff managers related to CRM trending data and 'lessons learned'.\nDevelops and presents education programs for personnel to enhance their awareness of their role in reducing medical errors to avoid patient injury and reduce liability exposure.\n\n

KEY SUCCESS FACTORS

\n\nBachelor's Degree in a Healthcare related field and four (4) years of experience in a clinical discipline or Associate's Degree in a Healthcare related field and six  (6) years of experience in a clinical discipline or RN if less than at the Bachelor's Degree level and six (6) years of experience in a clinical discipline.\nTX RN license if an RN\nCertified Professional in Healthcare Risk Management (CPHRM) preferred.\n\n
