How you will contribute (not limited to)

You will make a difference in our patient’s lives by coordinating with the assigned nurse to provide safe patient care and report out on the patient’s condition. You will perform personal care, take vitals, communicate with families, and other duties as assigned. You will travel between patients and work as part of an interdisciplinary team that supports both families and patients during their end-of-life journey. Competitive compensation (to include on-call pay when on rotation)Mileage reimbursementFlexible scheduleEmployee referral programTuition reimbursementPaid time off401(k) retirement savings planMedical, dental and visionLife insuranceFlexible Spending Account (FSA)Employee discountsCommunity Service Opportunities Responsibilities Performs personal care needs when given a patient's assignment and attends his/her requests promptlyReports to the Case Manager immediately, if unable to perform a certain taskProvides patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.Meets safety needs of patient and using equipment safely and properly (foot stools, side rails, O2)Gives personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assignedAssists in dressing and undressing patients as assignedPlans and prepares nutritious meals, including shopping, as assignedAssists in feeding the patient as assignedTakes and records oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency and when on the plan of careProvides proper care and observation of patient's skin to prevent breakdown of tissue over bony prominenceReports on patient's condition and significant changes to the Case ManagerAlso aware of the caregiver or other individuals living with the patient and interpersonal issuesAssists in ambulation and exercise as instructed by the hospice nurse or therapistProvides normal range of motion and transfers/positioning as directedAssists with self-administered medications as allowed by state regulations, such as reminders Offers and assists with bedpans and urinalsProvides assistance as assigned with light laundry needsPerforms range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competencyProvides respite for patient's/family/caregiver when on-site, as appropriateKeeps patient's living area clean and orderly, as assignedAdheres to the organization's documentation and care procedures and standards of personal and professional conductParticipates in quality assessment performance improvement teams and activitiesRespectful of patient and family/caregiver environment and patient's personal needs Qualifications Meet the training requirements of the state of practiceMinimum of six months experience including providing personal care At least 18 years of ageComplete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical trainingPossess and maintain current CPR certificationAbility to read and follow written instructions and document care givenUnderstands hospice philosophy, comfortable providing specialized care to the terminally illAbility to work with little direct supervisionAbility to handle death/dyingGood Organizational SkillsStrong Oral and written communication skills About our Line of Business Sacred Journey Hospice, and affiliate of BrightSpring Health Services, provides a special kind of care to patients living with advanced illnesses, their families, and their caregivers.
