The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals. Qualifications: Required: + High School Diploma or equivalent Preferred: + Bi-lingual in Spanish is strongly preferred Scheduling Requirements: + Full-Time, Evening + Working Location:NBR- New Brunswick, NJ Essential Functions: + Meet patients in a clinic, hospital, ER, and where patients are located and conduct needs assessment using Social Determinants of Health Tool. + Schedule weekly follow-up calls and home visits to patients. + Help patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members lives. + Assist patients with organizing their records, making follow-up appointments with a Primary Care Physicians or Specialists. + Help patients with the tasks of getting medical care, and work on health goals by helping to plan healthy meals or finding time to exercise. + Help patient s access resources, including quality of care, childcare services, food, housing, transportation. + Maintain relationship with the Community Based Organization. + Able to travel and meet the members where they at; may include local Churches, homeless shelter, and members residence. + Duties, responsibilities and activities may change at any time with or without notice, Other Duties: Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
