COORDINATOR HOME CARE

Hourly Pay Range: $19- $21 

 

SUMMARY OF DUTIES:

The Coordinator of Licensed Home Health performs a variety of duties both technical and clerical in nature. This position requires planning and organizational skills, the ability to investigate and resolve problems while providing technical assistance and support, along with the coordination of day to day office activities. Must have the ability to communicate well with various entities including: clinical staff, finance, consumers, attendants, doctors, and other departments in the building. The Coordinator of Licensed Home Health will support all Business Office functions to ensure the success of the unit and provide support to the clinical staff as needed.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

• Accepts all incoming referrals, confirms authorizations from contract agencies and ensures prompt placement of staff on new cases.
• Ensure accuracy of timesheets by reviewing the and entering into EHR.
• Follow up with any staff not following timesheet guidelines and document.
• Reconcile any lost billing hours by contacting field staff to complete missing or inaccurate time or activity sheets.
• Attend and participate in Quality Improvement meetings.
• Will participate in the audit process by preparing and presenting documents necessary to answer requests by regulatory bodies, corporate compliance and payers.
• Will participate in creation and revision of policy and procedure.
• Acts as a liaison between office and field staff to ensure compliance with scheduled inservices/trainings/attestations.
• Type, format, proofread, and edit correspondence and other documents from various sources, using Microsoft Office, including creation of presentations, meeting agendas/minutes as needed for Licensed Home Health.
• Assign, if appropriate and/or create and maintain current Licensed Home Health packets.
• Assign, if appropriate and/or maintain all current and closed filing for Licensed Home Health.
• Assign, if appropriate and/or maintains and updates all consumer prior authorizations for Licensed Home Health.
• Coverage of Home Health Specialist when needed to include distributing all incoming mail, retrieving and distributing all faxes to the correct individual.
• Assign, if appropriate and/or responsible for mailings for Licensed Home Health.
• Independently makes decisions regarding matching patient’s needs with staffs’ abilities.
• Serves as back up to other members of CHHA and will be cross trained to function in their position when necessary.
• Maintains neat and orderly work environment.
• Secures all HIPAA related information appropriately.
• Complies with all agency policies and procedures.
• Other duties as assigned.

MINIMUM QUALIFICATIONS:

• A Bachelor’s Degree and two years of experience in Human Services;
or
• Associates Degree and three years of experience in Human Services;
or
• Equivalent combination of education and experience;
and
• Experience working with persons with developmental and/or intellectual disabilities.
• Ability to handle multiple tasks simultaneously.
• Ability to interact with all levels of personnel.
