Job Details The Director of Care Management and Nursing operations is responsible for providing overall management of the Care Coordination department responsible for patient care coordination, including oversight of nursing Care Managers/Managers of Care Coordination, Social Workers, and office administrative staff. In addition the director will oversee bed management including the Hub and the nursing supervisors will report to them. The Director of Care Management and Nurse Operations will: Work collaboratively with, but not limited to, Medical Staff, Quality/Risk Management, CH Utilization and Central Appeals, Managed Care and Revenue Cycle and Patient Access departments to ensure the most advantageous clinical and financial outcomes. Support all CH and consultant initiatives addressing activities relative to Care Management and Denials. Identify opportunities for improvement that result in development of efficient processes and enhanced revenue. Provide guidance and education to the Care Coordination department, Medical Staff, and SCSH Administrative Staff. Ensure compliance with Hospital Conditions of Participation. Implement case management programs, including utilization review, intake or discharge planning. Evaluate patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards Develop and maintain budgetary departmental goals Perform department scheduling that works to maximize hospital coverage for discharge planning Address patient or family concerns/complaints Attend and address care coordination agenda items during intradepartmental meetings Support patient experience initiatives by promoting department iCare values and behaviors Be required to pursue continuing education, certification and self-development to remain current with industry standards and business objectives Ability to customize innovative discharge plans that supports frontline staff with challenging discharge barriers Coordinate with post-acute facilities (rehab, long term placement, assisted living, group home) to better support our patients oversee bed management including the Hub will oversee nursing supervisors What you’ll bring: Passion for Care Management and Leadership 3+ Years’ experience in a hospital care management leadership role is required Experience in utilization management, working with managed care organizations, and discharge planning is required Current knowledge of NY Department of Health and Centers of Medicare and Medicaid regulations is required Knowledge of the Joint Commission Standards, and federal, state and QIO regulations Familiarity with admission and concurrent in-patient criteria is preferred Experience with data analytics preferred Education: Bachelor of Science in Nursing (BSN) degree required Master of Science degree strongly preferred Licensure: New York Registered Nurse (RN) License & Registration Salary Range USD $160,000.00 - USD $200,000.00 /Yr.
