Discharge Planning Coordinator / RN
Oncology
Shift: Full Time 7:00 am - 5:30 pm
Location: Emory University Hospital
Contributes to excellence in patient/family centered care by coordinating the health care of select patient populations/designated services across the continuum of care using the nursing process.Promotes the safety and well being of all patients by coordinating the patient's discharge from the hospital and transition of care to the out-patient clinic. Collaborates with the inpatient interdisciplinary team to develop and maintain the plan of care. Works in collaboration with others to ensure that all discharge needs are met. Schedules all discharge appointments, treatments, and protocol specific orders.Completes needed discharge follow up phone calls to patients and documents appropriately. Performs other duties as required. Works with manager to formulate plan for professional development. Attends educational in-services as appropriate. Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments.
Minimum Qualifications:
Bachelors degree in Nursing. Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board. Three years of clinical nursing experience as a Registered Nurse with at least two years of recent experience (within the last five years) in the area of concentration. OCN certification preferred for some positions. BLS required; ACLS Certification preferred but may be required in some areas.
