Family Nurse Practitioner, Primary Care in Red Bank
Community Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care.
We are currently looking for a passionate Advanced Practice Nurse, to join our Primary Care Center team. In this full-time role, you would be responsible for providing holistic primary care services in collaboration with our Community Health Center team in Red Bank to help improve patient outcome and support the overall well being of the community. Responsibilities Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workersProvides direct family and patient centered services to primary care patients at the sites within established productivity guidelinesMaintains complete patient records using the Electronic Medical record and complies with all reporting mechanisms Ensures compliance with the New Jersey Department of Health Ambulatory Care Regulations for Licensure as well as requirements of other licensing and accrediting bodies, health maintenance organizations and other insurance carriersCollaborates in team based care in the Patient Centered Medical Home modelParticipates in Peer Review and other Quality Improvement activitiesCollaborates in the provision of in-service education for staffAdheres to CHC infection control and safety policies, and guidelines of regulatory agenciesParticipates in community outreach programs regarding CHC services and programsSupervises staff in conjunction with the organization’s managerial and leadership practicesOmission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position Qualifications Licensure as an Advanced Practice Nurse in NJMaster’s degree in Nursing from an NLN accredited programCertified as a Family Nurse Practitioner with prescriptive authority; maintain a CDS and DEA 2+ years of primary or ambulatory care advanced practice nursing experience preferred; pediatric experience preferred.Bilingual (Spanish/English) preferredCar and valid driver’s license
