Summary:
Care for a group of patients consistently using the nursing process and clinical standards and protocols. Responsible for managing the activities of the multi-disciplinary team providing care to the patient, in addition to monitoring the provision of therapeutic services through written and verbal communication with all disciplines. Ensure that all visits scheduled and provided are authorized and covered by physician orders. Responsibilities include review of patient visits weekly and providing plan to PCS and team assistant, including anticipated weekend visits. Inform team assistant of any schedule changes and communicates caseload and patient care issues to PCS.

Florida driver's license is required within 30 days of hire.
