SUMMARY: The Interdisciplinary Rehabilitation Coordinator is responsible for coordinating the provision of care, facilitating communication with persons served and their families, and is responsible for ensuring communication with external and internal sources. This position facilitates involvement of the persons served, gathering of information to assist with discharge/transition planning and implementation. Brings forward information to the team to assist with decision making from admission through discharge.
RESPONSIBILITIES:
Acts as intermediary with health insurance providers (HMO’s private insurance, Medicare, Medicaid), etc. to obtain approval for coverage for admission to the program and for ongoing coverage as documented in the record.
Act as liaison between referral sources and the program, as well and community resources, facilitating communication to ensure smooth transition throughout the continuum of care.
In collaboration with the interdisciplinary team develops and documents a comprehensive plan to address the needs identified through the comprehensive psychosocial assessment in compliance with professional, regulatory and facility standards.
In collaboration with the interdisciplinary team participates in daily rounds/outpatient team meetings developing care plans, delivering service and for discharge planning, including making appropriate referrals for medical, behavioral health, social and community needs of patients and their support systems.
Provides clinical assessment of patient’s psychosocial needs and determines appropriate interventions and strategies to meet those needs. Needs may include financial support or counseling, housing appropriateness, transportation and psychological supports
Involves patient/family in the treatment planning process as demonstrated by medical record documentation and signatures on appropriate documents.
Communicates information about the plan of care, educates and is a resource to other team members, patients and family regarding community resources.
Advises Director of quality issues which may arise within their respective department. Educates staff regarding reimbursement issues and business decisions related to patient programming. Facilitates discussion and decisions regarding patient programming to maximize resources for the patient and the health system.
Monitors staff compliance with recording of charges, completion of billing forms and patient documentation files to assure optimal reimbursement (Outpatient)
Monitors productivity and works cooperatively with other supervisors to maximize personnel resources across the health care system (Outpatient)
Facilitates and coordinates the development, updating and implementation of departmental policies and procedures, as well as, assists with intra and inter departmental quality improvement and care management initiatives.
Attends continuing education pertinent to supervisory skills, patient care, and/or program development. Makes recommendations for staff continuing education and career advancement. Acts as a role model in the promotion of professionalism, customer satisfaction, cooperation and problem solving.
Provides direction and training to support personnel as needed.
Reports problems and concerns and consults with Director of Clinical Services regarding clinical issues and decisions.
Fosters team, patient and family communication. Communicates pertinent patient information to all members of the health care team.
Displays the capacity for motivating others.
REQUIRED QUALIFICATIONS:
Minimum BA Degree in related field, eg. Social Work.
Master's Degree preferred at Golisano Restorative Neurology & Rehabilitation.
Minimum one year experience preferred.

14626The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
