Licensed Practical Nurse – Mobile Medication

The Program staff will treat participants with dignity and respect, helping them manage their medication and symptoms. Using a team approach, services will be available whenever needed. Staff will administer and monitor medications as prescribed, collaborating with the treatment team and Primary Care Physicians to address co-morbid medical conditions.

Depending on clinical area and training, may provide care and IV therapy following UPMC LPN Practice Guidelines and policies, and the State Board Regulations. The Licensed Practical Nurse (LPN) participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place under the direction of a Registered Nurse or other licensed UPMC provider.

Responsibilities:

Provide nursing care and judgment. Plan, implement, and evaluate patient care. Seek guidance and supervision when necessary. Follow UPMC policies and guidelines. Perform duties within LPN guidelines, including IVs. Set daily goals and plan holistic patient care. Communicate patient condition and document thoroughly. Improve patient safety, quality, and satisfaction. Educate patients and families for care transitions. Support team activities and continuous improvement.
