 $100,000 - $140,000 a year with a Base Salary and Compensation Model for Quality & Performance.

No weekend hours.   Our purpose is to enhance the life of each individual we touch by exceeding expectations through compassion and love.   Nurse Practitioner Job Responsibilities: The Advanced Practice Registered Nurse (APRN) participates as an active leader of the Skilled Nursing Facility (SNF) care team. The APRN works autonomously and in collaboration with the facility nursing and management staff, facility medical director, and the Exceptional Care Services (ECS) provider team to provide exceptional care to skilled and long-term care residents across all Exceptional Care Living Centers. The Nurse Practitioner is responsible for management of chronic health conditions and is expected to provide evidence-based care to all residents within the facility. Specific duties are as follows: Treat residents in a nursing facility during the work week and is on-site at the facility a minimum of eight (8) hours per workday providing on-site resident services. Attend care plan/family meetings as necessary. On-call and available via telephone from 7am-7pm Monday through Friday. Outside of these hours the staff will be directed to contact our contracted call service provider. Review labs, diagnostics, and consults for assigned residents. Review and initiate orders for all new admissions, readmissions, Emergency Room visits and outpatient/consult visits. Assess and treat both chronic and acute health conditions, to include prescribing new medications/treatments or adjusting existing regimens including controlled medications. Collaborate with contracted pharmacist to review and address all Pharmacy recommendations as appropriate. Initiate Goals of Care discussions related to code status, palliative care and end of life care, when appropriate. Collaborate with therapy and social services for discharge planning for short-term skilled patients. Maintain confidentiality of all patient records. Follow all federal, state, and nursing guidelines, regulations, and standards. Refer patients to specialists if necessary and analyzes medical records and reports. Know how to use popular tools, including: medical database software, medical research software, medical diagnosis software, medical billing software, e-prescribing software, scheduling software, billing software, and telemedicine software.   Nurse Practitioner Education and Experience Requirements: Active and unencumbered Nurse Practitioner and Registered Nurse licenses in the state of practice. National board certification as a Nurse Practitioner from an accredited agency, AANP or ANCC. Master's in nursing required, Doctor of Nursing preferred. Active prescriptive authority and DEA licensure. Two or more years of clinical experience as a nurse or nursing practitioner at a nursing home facility, physician practice or in a hospital setting with a related specialty is strongly preferred.
