Occupational Therapist - Home Health
This is a full time position, working Monday-Friday 8:00am-5:00pm PST
Compensation Band: $ to $ per hour
The occupational therapist contracted through the Organization is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement.Key Responsibilities: Evaluates patients’ functional abilities (e.g., muscle strength, coordination, communication, self-care, work capacity) and assesses the home environment for safety and independence. Identifies equipment needs and contributes to the overall care plan.Develops individualized treatment programs with clear functional goals, and communicates the plan to the physician and care team.Provides therapy to help patients regain or develop daily living skills, including use of adaptive equipment, splints, and prosthetic or orthotic devices.Performs work capacity evaluations and may refer patients to vocational training if returning to work is a goal, per organizational policy.Instructs patients, families, and care team members on treatment plans, equipment use, and techniques to support functional improvement.Maintains accurate clinical records and reports changes in patient condition to the physician, submitting documentation in line with organizational policy.Collaborates with other care team members to ensure coordinated, effective care, and participates in meetings, in-services, and case conferences as required.Supports discharge planning and identifies the need for additional home health services, making referrals as appropriate.Position Qualifications:Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.Registered with the National Registration Examination of the American Occupational Therapy Association.Two years of clinical experience as an occupational therapist.Possess and maintains current BLS Hands On CPR Certification.  $50 - $60 an hourSee what Mission has to offer!
