**Job Description**
**Responsible for coordinating activities relating to the delivery of goods and services, between the patient, nursing services, pharmacy, liaisons, and Central Intake.**
**Education And Credentials**
**Bachelors or Associates Degree in Nursing from an accredited school required. Current licensure to practice as a Registered Nurse in NYS. INS or ONS certification preferred.**
**Experience**
**Minimum of two years experience in Home Infusion. Previous experience in case coordination preferred. Ability to read, analyze and interpret professional journals, technical procedures, governmental regulations, and nursing periodicals and texts; Ability to effectively present information both verbally and in writing to managers, clients, healthcare workers, staff, customers, and the genreal public; Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals; Strong clinical knowledge and skills; excellent interpersonal communication, teaching and organizational skills. Basic computer skills desirable; Possess knowledge in aspects of certified and licensed Home Care Regulations; Knowledge of venous access devices and ability to insert peripheral intravenous lines and PIC lines; Possess knowledge relative to Infusion Therapy Standards of Practice.**

**Salary Range:** $42.23 -$52.68
