Position Summary
This is a 12 month pharmacy residency program in accordance to the standards determined by the American Society of Health-System Pharmacists. Comprehensive evaluation of the resident will be conducted through PharmAcademic™ by the residency program preceptors and residency program director. The resident must adhere to those standards and their contracts to satisfactorily graduate from the program at the discretion of the residency program director and the American Society of Health-System Pharmacists.
Primary Position Responsibilities
1.
In collaboration with health-care team, provide safe and effective patient care to diverse range of patients, including those with co-morbidities, high-risk medications, and multiple medications following consistent patient care process.
2.
Demonstrate ability to evaluate and investigate practice, review data, and assimilate scientific evidence to improve patient care and/or the medication use system.
3.
Provide effective medication and practice-related education to patients, caregivers and other health care professionals.
4.
Provide effective medication and practice-related education to students and the public (individuals and groups).

Minimum Education:

Doctor of Pharmacy Degree from ACPE-Accredited school of pharmacy (or one in the process of pursuing accreditation) OR have a Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate from the NABPPreferred:
