The role you’ll contribute:

The Performance Program Manager holds a critical role in shaping the infrastructure, processes, and technology that underpin provider performance management within our clinically integrated network and value-based enterprise. Reporting to the Executive Director of Provider Performance, this strategic role is dedicated to creating a sophisticated and proactive system that supports performance improvement initiatives, data-driven actions, and strategic planning across all providers for the state of Florida supporting the clinically integrated network and value-based enterprise.

The value you’ll bring to the team:

Performance Management Systems: Develop and maintain a robust system to manage provider performance, focusing on Quality Escalations, Utilization Escalations, Cost of Care Escalations, and Citizenship Escalations. Establish an effective process for Action Plan Management, ensuring timely and effective remediation strategies. Collaborate with executives and Medical Directors to establish provider accountability measures and implement into operational workflows.

Project Tracking and Collaboration: Implement and maintain a project tracking system for provider-facing teams to maintain accurate records of Remediation Management and Accountability. Progressive improvement strategies to enhance the sophistication and efficacy of performance management systems are expected to be generated and implemented as part of the evolving network needs. Collaborate closely with Network Development and Network Governance to integrate performance management systems with network operations. Collaborate with the Network Operations leadership as a key partner to system integration, clinical integration software, and CRM platforms.

CRM Architecture and Integration: In partnership with PHSO Operations team this role will oversee the CRM architecture (Salesforce) related to support the provider performance team's efficient workflow. Collaborate with Network Management to ensure accurate records within the CRM, facilitating seamless collaboration between teams.

Performance Programs System Management: Lead initiatives tracking and resolution related to Practice Transformation, Address Attribution Errors, and Incentive Management (Validation, distribution, calculation, analytics, collaboration with finance). Manage payer-specific performance programs, integrating applications and management into operational workflows such as HFHP MA CDI Bonus Distribution, Curation Health, and Stellar Health.

Enhanced Analytics and Strategic Planning: Leverage advanced analytical skills to derive insights from performance data. Leverage applications and tools such as Microsoft Office Suite, Tableau, and PowerBI in collaboration with the analytics team to create and deliver visualization tools that can impact performance management and translation of analysis to performance improvement with providers. Contribute to business analytics and strategic planning efforts to support data-driven decision-making.

The expertise and experiences you’ll need to succeed :

Minimum qualifications:

Bachelor's in healthcare administration, Business, or related field.

4 years of experience in healthcare.

Six Sigma Green Belt Certification.

Change Management Training.

Preferred qualifications:

Master's in healthcare administration, Business, or related field.

Experience with data-driven performance management and strategic planning

Familiarity with CRM architecture, provider incentives, and healthcare technology applications
