\nAs a Transplant Coordinator RN, utilize a nursing model for quality patient care. Assess patient needs and create personalized care plans. Watch how patients respond to personalized care. Ensure patients have smooth transitions through healthcare. Prevent complications or delays by understanding needs.
\nEssential Functions of the Role
\n\nAssessing transplant eligibility for patients involves interviewing them and their families.\nYou will coordinate tests for transplant candidates, identify issues in results, and arrange necessary care. You'll work closely with patients, physicians, and other healthcare providers during this time.\nManage recipient's post-transplant care.\nArrange admissions, lab tests, clinic visits.\nCoordinate with referring, primary care. Understand test results, identify irregularities.\nSet up needed care as directed.\nTeaching candidates, families, caregivers about evaluation, organ allocation, waiting, post-operative, immunosuppression effects, complications, and psychological impact of transplant.\nUsing insurance knowledge to help with post-transplant prescriptions. Assisting patients with coverage issues.\nMaintaining long-term prescriptions.\nComplying with various regulatory policies and maintaining records on the transplant waitlist through different online programs.\nCoordinating recipient transplants includes using the donor website and communicating with potential recipients. Assessing their health status and notifying hospital departments are important tasks. Updates are also sent to the team.\n\nKey Success Factors

Associate's degree level.(2) Two years of professional experienceAn (RN) Registered Nurse License.Basic Life Support (BLS) skills required or obtain within (30) thirty days if not already held.
