Unit Representative – Home Care/Hospice
Reports to: Clinical Team Manager
Job Qualifications/Experience:
+ High School Diploma, Bachelor’s degree preferred
+ At least three years administrative experience or a graduate of an accredited business school with at least one-year administrative experience
+ Ability to use office equipment efficiently including computer proficiency
+ Ability to work in a team setting
+ Highly organized
+ Demonstrates excellent communicative and customer service skills
+ Ability to identify problems and seek the appropriate assistance to rectify them.
+ Ability to work with diverse group of colleagues and customers at all levels of the organization.
Duties & Responsibilities:
+ Assists with team members scheduling including weekends:
+ Assigns scheduled visits to appropriate team members
+ Communicates with clients to confirm visit dates and times for visits
+ Collaborates with answering service to provide on-call triage assignment including updates throughout the month.
+ Maintains calendars for team members schedules:
+ Prepares and distributes monthly schedules
+ Maintains accurate electronic schedule in EPIC.
+ Manages communications for assigned clinical team:
+ Receives and delegates phone calls, letters and faxes to appropriate team member
+ Effective communication to facilitate optimal customer service
+ Maintains confidentiality.
+ Works in collaboration with all agency team members:
+ Supports other clinical teams based on agency need
+ Flexes work hours to ensure office coverage
+ Accepts other assignments as requested/assigned.
+ Manages various reports with appropriate follow up with field clinicians, Clinical Assistant Managers and Team Manager.
+ Orients new employees:
+ Cross trains to learn new skills and demonstrates a willingness to teach other.
+ Assists with management of IDT and staff meetings:
+ Assists with management of team meeting calendar
+ Assists with scheduling and managing annual competencies and mandatory in-services
+ Coordinates services for outside labs, tracks lab results and faxes to field nurse and physician
+ Provides lab results to clinical team Assistant Manager for review
+ Coordinates and facilitates DME orders including faxing request to MD for signature and communicating with DME company regarding progress of order.
